Report a Concern
We encourage you to report your patient care and safety concerns to your caregiver, department director, patient representative or hospital administration. Our hospital supervisor is available to assist you after-hours and on weekends.
If we are not able to provide a satisfactory resolution, you also have access to a number of outside organizations, either directly or through the patient representative:

Medicare beneficiaries have the right to request a review by the state peer review organization, CFMC, for quality of care, Medicare written Notice of Non-coverage, or premature discharge concerns. This can be arranged through an Exempla Patient Representative or one of Exempla's Case Managers.

Residents or family members of sub acute units may contact the Colorado Ombudsman Program to report any issue related to their hospital stay.
